We bill all insurance plans that we participate with. It is the patient’s responsibility to pay copays, coinsurance, and deductibles that are due at the time of service. The patient is responsible for obtaining a referral from the Primary Care physician if required, before they can be seen. It is the patient’s responsibility to provide us with your current insurance, and to notify us if anything changes during your course of treatment.
Please note that if your visit is related to an accident of any kind you must contact your insurance company with accident details or your claim may be denied or delayed. Please contact your Insurance provider if you have questions about coverage or benefits. Remember that we are only the medical provider and have no ability to negotiate on your behalf.
